ANNUAL FORMS

Annual forms are used to calculate funding for schools and provide benefits to families. The Educational Benefits form on PowerSchool determines a large portion of Fairmont's funding. Without full participation, our school loses access to thousands of dollars - we need you! It only takes a couple of minutes!

Note: If you are new to Fairmont, you likely do not have your PowerSchool login yet. You will receive this when your child comes to school. Please plan to complete those forms within the first couple days of school - they are VERY important!

COMPLETE YOUR EDUCATIONAL BENEFITS FORM ONLINE

On PowerSchool, under "Forms" you will fill out a series of 7 forms under "Annual" forms. After submitting the Signature page, you must return to the main "Forms" page to select and complete the "25-26 Education Benefits Form." A green check mark labeled "Submitted" means you're done!

IMPORTANT: The top of the form mistakenly says, "Please submit one form per household." The district has confirmed, you must complete the form for each child in your household.

Having trouble getting into PowerSchool or completing your Educational Benefits form online? Print a paper copy of the form and submit to Luisa in the Fairmont office during school hours.
